WHO IS ELIGIBLE
All persons who need continuing care in their own homes for the purpose of minimizing the effects of illness, injury/accident or disability. Services are provided to all clients regardless of race, age, sex, creed, color, national origin, religion, handicap and marital status.
Are you caring for a loved one who:
- Lives alone and needs assistance
- Needs geriatric specialty care
- Needs assistance with Activities of Daily Living (ADL)
- Not able to get in and out of bed, a tub, a wheelchair or trouble getting around alone.
- Have family living far away or lacks adequate support systems
- May be disabled or in wheelchair
- Needs assistance with developmental/situational crisis
- Is recovering from an illness or hospital stay
- Is at risk for having complications
- Needs health teaching and/or assistance with health conditions and treatments
- Has Alzheimer’s/Dementia
- Has Cardiac Disease
- Has Congested Heart Failure
- Has Parkinson’s Disease
- Wants to avoid frequent trips to the ER
